The sliding fee discount varies, based on the patient’s annual household income levels and how many dependents a patient has. View the current Federal Poverty Guidelines (FPG).

Yes. We encourage patients to apply for our Sliding Fee Discount Program, even if you have insurance.  The Sliding Fee Discount Program can be applied to out of pocket expenses, not covered by your insurance.

We recommend you call your insurance company before making your first appointment to find out if Noble Community Clinics is in-network.

Not only do they help you apply for our Sliding Fee Discount Program, but they can also help you apply for:

• Health Insurance through the marketplace
• Medicaid (including BadgerCare)
• Medicare
