Important Information for Lakeshore Patients
The Manitowoc, Sheboygan and West Bend clinics are now operating as Noble Community Clinics. This page will help you find important information related to your previous Lakeshore care. Below, you’ll find information regarding billing, MyChart access, updated Sliding Fee Scale details, and more.
Providers in Your Insurance Network
You may receive a letter stating your provider is out of network due to Lakeshore’s transition to Noble Community Clinics. Please don’t worry—under Noble, your provider should still be in your insurance network.
We will continue to care for all patients, including those with Medicaid, private insurance, and those without insurance. If you have questions, feel free to contact our office.
Do you need to pay for services you received before April 1?
If you received services at Lakeshore Community Health Care before April 1, you may still have an outstanding balance associated with that care. Click here to make a payment and/or inquire about services provided prior to the integration.

Box 1440, Wautoma, WI 54982, Attn: HIPAA Compliance Officer, 920-787-9461, nobleclinics.org, for at least the period of time required by applicable law.
