The Manitowoc, Sheboygan and West Bend clinics are closed until Thursday, April 2, when Lakeshore will begin operating as Noble Community Clinics. This page will help you find important information related to your previous Lakeshore care. Notice is hereby given in accordance with Wisconsin Statute Sec. 146.819 that after March 31, 2025 all the patient medical and dental records of Lakeshore Community Health Care, Inc. will be maintained by La Clinica de los Campesinos, Inc., doing business as Noble Community Clinics, a Wisconsin not for profit corporation, at 400 S Townline Road, P.O. Box 1440, Wautoma, WI 54982, Attn: HIPAA Compliance Officer, 920-787-9461, nobleclinics.org, for at least the period of time required by applicable law.
