What is a Community Health Worker
The role of our Community Health Worker here at Noble is to connect patients with needed resources to help support their overall health and well-being.
Types of Resources
Community Health Workers will look to connect patients with any internal resources that Noble has to offer as well as local, county, state, and federal programs. This can be related to transportation, utilities, food assistance, prescription cost savings, and many other resources that can influence the patient’s overall health.
What to Expect
- The Community Health Worker will utilize a Social Determent of Health Screener. This will help identify what resources to connect you with.
- The Community Health Worker will review the screener results and discuss options with you in a 30-minute meeting.
A follow-up meeting can be scheduled if needed. - The Community Health Worker will offer to connect you directly with the identified resources. If you would like to follow-up for yourself, the CHW can send the information to you with the resource and explain details on what should be prepared for the follow-up.
- All information received will remain confidential.
